Анотація

Актуальність: Хронічний риносинусит (ХРС) є одним із найпоширеніших захворювань верхніх дихальних шляхів, що вражає 5-15% населення у різних регіонах світу. Незважаючи на десятиліття досліджень, точний патогенез ХРС залишається предметом дискусій, а роль мікробіому носової порожнини потребує детального вивчення.

Мета дослідження: Проаналізувати та систематизувати сучасні дані (2018-2025 рр.) щодо ролі мікробіому носової порожнини та приносових пазух у патогенезі хронічного риносинуситу та провести їх метааналіз.

Матеріали та методи: Проведено систематичний пошук в базах даних PubMed, Scopus, Web of Science, Cochrane Library та Google Scholar. Відібрано 21 дослідження, що відповідали критеріям включення. Оцінка якості досліджень проводилась за шкалою Newcastle-Ottawa для обсерваційних досліджень та інструментом Cochrane Risk of Bias 2 для рандомізованих контрольованих досліджень. Метааналіз виконано з використанням моделі випадкових ефектів. Якість доказів оцінено за системою GRADE.

Результати: Метааналіз виявив статистично значуще підвищення поширеності Haemophilus influenzae у пацієнтів з ХРС (OR: 2,00; 95% ДІ: 1,08-3,72; p=0,0276) та зниження відносної частоти Corynebacterium spp. (різниця середніх: -5,44%; 95% ДІ: -8,88 – -2,00; p=0,0019). Встановлено значуще зниження показників бактеріального різноманіття у пацієнтів з ХРС (SMD: -0,62; 95% ДІ: -0,90 – -0,34; p<0,0001). Не виявлено статистично значущих відмінностей у поширеності Staphylococcus aureus між пацієнтами з ХРС та контрольною групою, проте встановлено сильну позитивну кореляцію між рівнем Staphylococcus aureus та прозапальним цитокіном IL-8 (r=0,67; 95% ДІ: 0,55-0,76; p<0,0001).

Висновки: Проведений метааналіз підтверджує концепцію мікробного дисбіозу як важливого патогенетичного механізму хронічного риносинуситу. Зниження бактеріального різноманіття, підвищення поширеності Haemophilus influenzae та зменшення вмісту коменсальних Corynebacterium spp. є характерними ознаками змін мікробіому при ХРС. Різні фенотипи ХРС характеризуються специфічними мікробними профілями, що підкреслює важливість персоналізованого підходу до діагностики та лікування.
